New Patients

Paynesville Medical Centre welcomes new patients. All new patients are required to complete a New Patient Registration Form and pay a registration fee of $45. Your first appointment will be with our nurse who will prepare your clinical record and update all baselines measures that are required for good health maintenance. From there you will be able to book appointments online for any of our GPs.

To protect your personal information in the form will clear and reset after 1 minute of inactivity. We suggest you complete the form in a distraction-free environment to avoid having to re-complete the form.
