We understand how challenging it can be living with a chronic disease
GPs can plan and coordinate the healthcare of patients with chronic conditions, including those that require care from multiple practitioners.
A GP Management Plan, also known as a Care Plan, can be created by our doctors for patients with chronic conditions such as asthma, diabetes, heart disease, arthritis, and other conditions.
A Care Plan serves as a central record for all the information about how to manage your chronic condition.
With a Care Plan, you may be eligible to see an allied health care provider i.e. (physiotherapist, podiatrist, dietitian, exercise physiologist, chiropractor, and osteopath) which may be partly covered by Medicare.
With a Care Plan, you'll also have the opportunity to contribute to the decisions surrounding what treatments and medical providers you would prefer as well as what health goals you'd like to achieve in the long term.
